ABSTRACT

Investigaciones de las últimas décadas revelaron que un ambiente adverso en la etapa de desarrollo puede producir una mayor susceptibilidad hacia fenotipos relacionados con enfermedad cardiovascular, diabetes, hipertensión, trastornos neuroconductuales y otras enfermedades crónicas no transmisibles. Estas enfermedades, cuyo aumento ocurre especialmente en países con alta vulnerabilidad social, provocan muertes prematuras y constituyen la primera causa de muerte en la vida adulta, además de un elevado costo para la salud pública. Consciente de la necesidad de prevenir estas enfermedades desde los primeros mil días de vida, la Sociedad Argentina de Pediatría creó la Subcomisión DOHaD y formuló una declaración para la prevención de enfermedades no transmisibles a la que adhirieron otros países de Latinoamérica. La aplicación de las estrategias declaradas con acciones interdisciplinarias e intersectoriales sostenidas en el tiempo contribuirá a construir salud, a disminuir la carga de enfermedades crónicas no transmisibles y al mayor bienestar y productividad para los pueblos.


Research in recent decades has revealed that an adverse environment in the developmental stage can produce a greater susceptibility to phenotypes related to cardiovascular disease, diabetes, hypertension, or neurobehavioral disorders, among other chronic noncommunicable diseases. These diseases, whose tendency is increasing especially in countries with high social vulnerability, cause premature deaths and constitute the first cause of death in adult life as well as a great cost to public health. Aware of the need to prevent these diseases from the first thousand days of life, the Sociedad Argentina de Pediatría created the DOHaD Committee and formulated a statement for the prevention of NCDs, to which Latin American countries also adhered. We believe that the application of the declared strategies with interdisciplinary and intersectoral actions sustained over time will contribute to building health, reducing the burden of NCDs, and to greater wellbeing and productivity for the people

ABSTRACT

Fundamento: Las alteraciones del estado nutricional materno generalmente se relacionan con desviaciones del crecimiento fetal, que pueden detectarse por los parámetros biofísicos fetales e identifican la posible condición trófica al nacer. Objetivo: Determinar la posible relación entre los parámetros biométricos fetales, la condición trófica al nacer y el producto de acumulación de los lípidos. Metodología: Se realizó un estudio transversal en el Policlínico Chiqui Gómez Lubian del municipio Santa Clara, durante el año 2019, en una población de 253 gestantes normopeso supuestamente sanas al inicio de la gestación. La muestra no probabilística fue de 144 gestantes. Las variables de estudio fueron: producto de acumulación de los lípidos, biometría fetal y condición trófica al nacer. Se utilizaron métodos teóricos, empíricos y estadísticos. Resultados: En el segundo trimestre ningún parámetro biométrico coincidió con la condición al nacer de pequeño, mientras que para el grande coincidieron las circunferencias cefálica y abdominal. En el tercer trimestre la longitud del fémur y la circunferencia abdominal coinciden en la identificación del pequeño y del grande. El PAL se correlacionó con la circunferencia abdominal del tercer trimestre y con el peso al nacer; presentando mayor frecuencia de valores en el tercer tertil para los nacimientos grandes. Conclusiones: La circunferencia abdominal fue el parámetro biométrico con mayor coincidencia con la condición trófica al nacer, la que se asoció con valores en el tercer tertil del PAL para la detección de nacimientos grandes, relacionándose el fenotipo normopeso metabólicamente obeso con el crecimiento fetal por exceso.


Background: Maternal nutritional status disorders are usually related to fetal growth deviations, which can be detected by fetal biophysical parameters and identify the possible trophic condition at birth. Objective: To determine the possible relationship between fetal biometric parameters, the birth trophic state and lipid accumulation product. Methodology: A cross-sectional study was conducted at the Chiqui Gómez Lubian Polyclinic in Santa Clara municipality, during 2019, in a population of 253 normal-weight pregnant women who were apparently healthy at the beginning of their gestation. The non-probability sample was made up of 144 pregnant women. Study variables were: lipid accumulation product, fetal biometry and trophic condition at birth. Theoretical, empirical and statistical methods were used. Results: In the second trimester, none of the biometric parameters matched the condition at birth as a small child, while in the large one the head and abdominal circumferences matched. In the third trimester, femoral length and abdominal circumference coincide in identifying the small one and the large one. LAP correlated with third trimester abdominal circumference and birth weight, presenting higher frequency of values in the third tertile for large births. Conclusions: Abdominal circumference was the biometric parameter with the highest coincidence with trophic condition at birth, associated with values in the third tertile of the LAP for detecting large births, relating the metabolically obese normal weight phenotype with excessive fetal growth.

ABSTRACT

Los movimientos fetales son uno de los primeros signos de vitalidad fetal. Durante la gestación, éstos van apareciendo progresivamente. La adecuada adquisición y mantenimiento de los mismos durante la gestación indica un correcto desarrollo neuromuscular, así como de bienestar fetal1. La percepción materna de una Disminución de los Movimientos Fetales (DMF) constituye un motivo de consulta frecuente en los Servicios de Urgencias Obstétricas; toda paciente embarazada debe vigilar los movimientos fetales, mediante un conteo subjetivo de los movimientos del feto, a partir de las 24 semanas de gestación. La DMF constituye el 5 ­ 15% de motivos de consulta en los servicios de Urgencias en el tercer trimestre del embarazo. Hasta un 25% de fetos que presentan una DMF presentarán alguna complicación perinatal (malformaciones, retraso de crecimiento, parto prematuro, hemorragia fetomaterna, y éxitus fetal) incluso en población de bajo riesgo. El manejo inadecuado de la DMF representa un 10-15% de las muertes evitables a término1-3. Es por esto que ninguna paciente que consulte por Disminución de Movimientos Fetales debe ser dada de alta sin asegurarse del adecuado bienestar fetal.


Fetal movements are one of the first signs of fetal vitality. During gestation, they appear progressively. Adequate acquisition and maintenance of fetal movements during gestation indicates correct neuromuscular development, as well as fetal well-being1. Maternal perception of decreased fetal movements (DMP) is a frequent reason for consultation in Obstetric Emergency Departments; every pregnant patient should monitor fetal movements by subjectively counting fetal movements, starting at 24 weeks of gestation. FMD constitutes 5-15% of the reasons for consultation in the emergency department in the third trimester of pregnancy. Up to 25% of fetuses with FMD will present some perinatal complication (malformations, growth retardation, premature delivery, fetomaternal hemorrhage, and fetal death) even in low-risk populations. Inadequate management of FMD accounts for 10-15% of preventable deaths at term1-3. This is why no patient who consults for decreased fetal movements should be discharged without ensuring adequate fetal well-being.

Objetivo. Establecer la utilidad de la medición de la longitud del riñón fetal en la predicción de la edad gestacional. Diseño. Estudio de cohortes, prospectivo y longitudinal. Institución. Hospital Central "Dr. Urquinaona", Maracaibo, Venezuela. Participantes. Mujeres con embarazos simples de riesgo bajo, entre 18 y 40 semanas de gestación. Métodos. Se midieron los valores de diámetro biparietal, circunferencia abdominal, longitud del fémur y longitud del riñón fetal durante la duración del embarazo. Principales medidas de resultado. Predicción de la edad gestacional por medición de la longitud del riñón fetal. Resultados. Se seleccionó los datos de 215 gestantes. Se realizaron un total de 3,291 evaluaciones, siendo el menor número de evaluaciones a las 31 semanas con 128, y el mayor número 157, a las 28 semanas. La longitud del riñón fetal presentó correlaciones fuertes, positivas y significativas con la edad gestacional por fecha de última menstruación y por las mediciones ecográficas (p < 0,001). El modelo de edad gestacional predicha por el diámetro transverso del cerebelo alcanzó un valor del coeficiente de determinación de 0,682. La correlación entre la edad gestacional por fecha de última menstruación y la predicha por el modelo alcanzó un valor de r = 0,826 (p < 0,001). Conclusión. La medición de la longitud del riñón fetal es útil para predecir la edad gestacional y junto a otras mediciones ecográficas rutinarias puede mejorar la capacidad de los modelos de predicción actuales.


Objective: To establish the usefulness of fetal kidney length measurement in the prediction of gestational age. Design: Prospective, longitudinal cohort study. Institution: Hospital Central "Dr. Urquinaona", Maracaibo, Venezuela. Participants: Women with low-risk singleton pregnancies, between 18 and 40 weeks of gestation. Methods: Biparietal diameter, abdominal circumference, femur length, and fetal kidney length were measured during the duration of pregnancy. Main outcome measures: Prediction of gestational age by measurement of fetal kidney length. Results: Data from 215 pregnant women was selected. A total of 3,291 total evaluations were performed, with the lowest number of evaluations at 31 weeks (128), and the highest number (157) at 28 weeks. Fetal kidney length presented strong, positive and significant correlations with gestational age by date of last menstrual period and by ultrasound measurements (p < 0.001). The model of gestational age predicted by the transverse diameter of the cerebellum reached a value of the coefficient of determination of 0.682. The correlation between gestational age by date of last menstrual period and that predicted by the model reached a value of r = 0.826 (p< 0.001). Conclusion: Measurement of fetal kidney length is useful for predicting gestational age and together with other routine ultrasound measurements may improve the ability of current prediction models.

Métodos : Se midieron los valores de diámetro biparietal, circunferencia abdominal, longitud del fémur y diámetro transverso del cerebelo fetal durante la duración del embarazo. Principales medidas de resultado. Predicción de la edad gestacional por medición de diámetro transverso del cerebelo. Resultados : Fueron seleccionados los datos de 215 embarazadas. Se realizaron un total de 3,858 evaluaciones totales, siendo el menor número de evaluaciones 131 a las 18 semanas y el mayor número 157 a las 28 semanas. El diámetro transversal del cerebelo presentó correlaciones fuertes, positivas y significativas con la edad gestacional por fecha de última menstruación y las mediciones ecográficas (p < 0,001). El modelo de edad gestacional predicha por el diámetro transverso del cerebelo alcanzó un valor del coeficiente de determinación de 0,908. La correlación entre la edad gestacional por fecha de última menstruación y la predicha por el modelo alcanzó un valor de r = 0,953 (p < 0,001). Conclusión : La medición del diámetro transversal del cerebelo es un parámetro útil para predecir la edad gestacional en embarazadas sanas.

Introdução: Durante o desenvolvimento do encéfalo há formação de sulcos e giros que podem sofrer alterações morfológicas similarmente ao nervo facial (NC VII) que possui funções sensitivas e motoras e sua lesão pode gerar prejuízos clínicos e estéticos. Na literatura há informações sobre essas estruturas em adultos, porém pouco definidas em fetos. Objetivo: Descrever os giros e sulcos e o trajeto do NC VII em cadáveres de fetos humanos. Métodos: Selecionou-se sete fetos humanos, de ambos os sexos, do acervo do Laboratório de Anatomia da UFMS CPTL. Realizou-se três etapas: avaliação da idade fetal, dissecação e descrição anatômica. O estudo foi aprovado pelo Comitê de Ética em Pesquisa, CAEE: 75069617.5.0000.5386 2022. Resultados: observou-se na face dorsolateral sulcos frontal superior e médio e pós-central, temporal superior e inferior, circular da ínsula e central da ínsula. Há giro pré-central, pós-central, temporais, superior inferior e médio. Na face medial há sulco do cíngulo duplo, paracentral, parieto occipital e giro frontal superior. Em todos os fetos, há o tronco do NC VII e seus ramos perfurando o parênquima da glândula parótida na região infratemporal antes da divisão em ramos pela face Conclusão: A topografia dos giros e sulcos dos fetos é fundamental para identificar precocemente malformações corticais. Somado a isso, o estudo morfológico do NC VII poderá fomentar estudos futuros, haja vista que são poucos os registros a respeito das características deste nervo em fetos.


Introduction: In the development of the brain there are grooves and gyri that undergo morphological changes, similarly the facial nerve (CN VII) has sensory and motor functions and its injury generates clinical and aesthetic damage. In the literature there is information about these structures in adults, but little defined in fetuses. Objective: To describe the gyri and sulci and the course of CN VII in fetal cadavers. Methods: Seven human fetuses of both sexes were selected from the collection of the Anatomy Laboratory of the UFMS CPTL. Three steps were performed: assessment of fetal age, dissection and anatomical description. The study was approved by the Research Ethics Committee, CAEE 75069617.5.0000.5386. Results: in the studied brains, superior and medium frontal and postcentral, superior and inferior temporal, insula circular and insula central grooves were observed on the dorsolateral surface. There are precentral, postcentral, temporal, superior, inferior, and middle gyrus. On the medial surface there is the double cingulate sulcus, paracentral, parieto occipital and superior frontal gyrus. In all fetuses, there is the trunk of CN VII and its branches piercing the parotid gland parenchyma in the infratemporal region before dividing into branches across the face. Conclusion: The topography of the gyri and sulcus of fetuses is essential for early identification of cortical malformations. Added to this, the morphological study of CN VII may encourage future studies, given that there are few records regarding the characteristics of this nerve in fetuses.


Introducción: En el desarrollo del cerebro existen surcos y circunvoluciones que sufren cambios morfológicos, de igual manera el nervio facial (NC VII) tiene funciones sensoriales y motoras y su lesión genera daño clínico y estético. En la literatura existe información sobre estas estructuras en adultos, pero poco definidas en fetos. Objetivo: Describir las circunvoluciones y surcos y el curso del NC VII en cadáveres fetales. Métodos: Siete fetos humanos, de ambos sexos, fueron seleccionados de la colección del Laboratorio de Anatomía de la UFMS CPTL. Se realizaron tres pasos: evaluación de la edad fetal, disección y descripción anatómica. El estudio fue aprobado por el Comité de Ética en Investigación, CAEE 75069617.5.0000.5386. Resultados: en los cerebros estudiados, se observaron surcos en la superficie dorsolateral frontal superior y medio y poscentral, temporal superior e inferior, insular circular e insular central. Hay circunvolución precentral, poscentral, temporal, superior, inferior y media. En la superficie medial se encuentra el doble surco cingulado, paracentral, parieto occipital y giro frontal superior. En todos los fetos, el tronco del NC VII y sus ramas perforan el parénquima de la glándula parótida en la región infratemporal antes de dividirse en ramas a lo largo de la cara. Conclusión: La topografía de las circunvoluciones y surcos de los fetos es fundamental para la identificación temprana de malformaciones corticales. Sumado a esto, el estudio morfológico del NC VII puede alentar futuros estudios, dado que existen pocos registros sobre las características de este nervio en fetos.

ABSTRACT Introduction: Congenital heart diseases (CHDs) constitute the most prevalent congenital pathology, and they are a consequence of structural and functional abnormalities during fetal development. The etiology of CHD involves the interaction of genetic and environmental factors. Fetal cardiac surgery aims at preventing natural pathways of CHD in utero, mitigating progression to more complex abnormalities. The goal of this review was to demonstrate the benefits and risks of fetal interventions in the two most prevalent CHDs, pulmonary stenosis and pulmonary atresia with an intact ventricular septum, but also critical aortic stenosis and hypoplastic left heart syndrome. Methods: Original and relevant articles were selected by meta-aggregation to perform a qualitative analysis of fetal cardiac interventions for pulmonary stenosis and critical aortic stenosis. The Joanna Briggs Institute's Qualitative Assessment and Review Instrument (or JBI-QARI) was used for data quality appraisal. Results: Of 61 potential articles, 13 were selected, and nine were finally included. Discussion: The present review demonstrated that fetal cardiac surgery increases right ventricular growth and hemodynamic flow in pulmonary stenosis, whereas in critical aortic stenosis it enables growth of the left ventricle and increases left ventricular pressure. However, it has a high complication rate, along with considerable morbidity and mortality. Conclusion: The benefits of fetal cardiac surgery for pulmonary stenosis and critical aortic stenosis are well-described in the literature; however, there is a significant risk of complications which can be reduced by the surgeon's technical expertise and well-structured hospital facilities.

Objective:To investigate the significance of abnormal morphology of Sylvian fissure detected by fetal neurosonogram (NSG) in prenatal diagnosis of malformations of cortical development (MCD).Methods:This retrospective study involved fetuses with abnormal morphology of Sylvian fissure on prenatal NSG in Peking University First Hospital between January 2016 and December 2021. Clinical data including the basic information as well as the results of NSG, genetic examinations and MRI were collected. The diagnosis of MCD could be made when both brain morphological abnormalities and pathogenic/likely pathogenic genetic abnormalities were presented. The association between the abnormal morphology of Sylvian fissure and MCD was analyzed by descriptive analysis.Results:Thirteen participants who had complete genetic information were included in this study [defined as those who were found with pathogenic/likely pathogenic copy number variation (CNV) or those who further underwent whole-exome sequencing (WES) as no pathogenic/likely pathogenic CNV were detected]. Twelve fetuses (12/13) were eventually diagnosed with MCD. Pathogenic CNV were found in seven fetuses and pathogenic point mutations in five, involving six pathogenic genes and four genetic syndromes. Symmetric morphologic abnormality of Sylvian fissure was detected in 10 cases by prenatal NSG with shallow and broad shape in six and abnormal angle of Sylvian fissure in four. The other two fetuses showed asymmetric abnormal morphology of Sylvian fissure that was shallow and broad shape on one side and abnormal angle on the other. The imaging features of MCD present by prenatal NSG and were consistent with those of MRI.Conclusions:Abnormal morphology of Sylvian fissure detected by prenatal NSG is important in MCD diagnosis. Genetic examination are recommended to the fetuses with abnormal morphology of Sylvian fissure. For those requiring for genetic analysis, chromosomal microarray analysis together with WES might be an optimal choice.

Abstract Maternal obesity has been described as a clinical entity associated with an increased incidence of metabolic diseases in the offspring, indicating a fetal programming phenomenon during this critical development period. Fetal exposure to an obesogenic environment affects multiple organs and tissues, including skeletal muscle, which is particularly susceptible to stressors from the external environment. Several studies have described alterations in the morphology and composition of skeletal muscle tissue secondary to obesogenic exposure in utero. In addition, modifications in signaling pathways related to the metabolism of energy substrates have been found in children born to mothers with obesity during pregnancy. This review addresses the current evidence describing the consequences of fetal exposure to an obesogenic maternal diet on skeletal muscle tissue, focusing on changes in tissue composition, alterations in signaling pathways related to glucose and fatty acid metabolism, mitochondrial biogenesis, and oxidative phosphorylation.


Resumen La obesidad materna se ha descrito como una entidad clínica asociada con el aumento en la incidencia de enfermedades metabólicas en el producto de la gestación, lo que indica la existencia de un fenómeno de programación fetal que se lleva a cabo durante este periodo crítico del desarrollo. La exposición del feto a un ambiente obesogénico afecta múltiples órganos y tejidos, incluyendo el músculo esquelético, el cual es particularmente susceptible a estresores del ambiente externo. Diversos estudios han descrito alteraciones en la morfología y composición del tejido muscular esquelético secundarias a una exposición obesogénica in utero. Además, se han encontrado modificaciones en vías de señalización relacionadas al metabolismo de sustratos energéticos en los productos de madres con obesidad durante la gestación. En la presente revisión se aborda la evidencia actual que describe las consecuencias de la exposición fetal a una dieta materna obesogénica sobre el tejido muscular esquelético, con especial enfoque en los cambios en la composición del tejido, las alteraciones en las vías de señalización relacionadas con el metabolismo de la glucosa y los ácidos grasos, así como la biogénesis mitocondrial y la fosforilación oxidativa.

RESUMEN Introducción: Las tablas de crecimiento y la ecografía de rutina son el estándar de oro para valorar adecuadamente al feto. Se consideran óptimas y adecuadas las tablas de biometría fetal que se crean con datos propios de cada población con el fin de usarlas como referencia, para identificar oportunamente malformaciones, o desviaciones del crecimiento. Objetivo: Identificar desviaciones de la norma de incremento en variables biométricas: diámetro biparietal, circunferencia cefálica, circunferencia abdominal y longitud del fémur. Métodos: Se realizó una investigación con diseño analítico longitudinal retrospectivo en la provincia Villa Clara, en el período comprendido entre enero de 2013 a diciembre de 2017. La población de estudio estuvo conformada por 6050 gestantes. La selección de la muestra se realizó a través de un muestreo no probabilístico intencional por criterios y quedó constituida por 3910 gestantes. Se obtuvieron datos de libros de registros de consultas de genética de áreas de salud seleccionadas. Se utilizaron medidas de resumen para variables cuantitativas, medias y percentiles (10, 50 y 90). Resultados: En el diámetro biparietal, la circunferencia cefálica y longitud del fémur, los valores reales obtenidos fueron inferiores a Hadlock en la mayoría de las semanas; la circunferencia abdominal cambió el comportamiento que llevaban las variables, ya que en esta predominaron los valores superiores entre los reales obtenidos, sobre todo en el percentil 10. Conclusiones: Los percentiles bajos de todas las variables en las primeras semanas, presentaron valores superiores a los de la tabla de Hadlock, pero en la circunferencia abdominal fetal, la muestra local tiene valores superiores mayoritarios a través de las semanas.


ABSTRACT Introduction: growth charts and routine ultrasound are the gold standard to adequately assess the fetus. Fetal biometry tables, created with data from each population, are considered optimal and appropriate in order to use them as a reference and to timely identify malformations or growth deviations. Objective: to identify deviations from the increase norm in the following biometric variables: biparietal diameter, head circumference, abdominal circumference and femur length. Methods: a retrospective, longitudinal and analytical study was conducted in Villa Clara province between January 2013 and December 2017. The study population consisted of 6050 pregnant women. The selection of the sample was carried out through an intentional non-probabilistic sampling and was made up of 3910 pregnant women who meet the criteria established for the study. Data were obtained from genetic consultation record books from selected health areas. Summary measures were used for quantitative variables, means and percentiles (10, 50 and 90). Results: the real values obtained in the biparietal diameter, head circumference and femur length were lower than Hadlock in most weeks; abdominal circumference changed the behavior of the variables, since in this the superior values prevailed between the real ones obtained, mainly in the10th percentile. Conclusions: low percentiles of all variables in the first weeks had higher values than those of the Hadlock table, but in the fetal abdominal circumference, the local sample had higher values throughout the weeks.

ABSTRACT

Backgroung: Neuroscientific research has provided great discoveries regarding the understanding of the brain functioning and its neural circuits. With advances in studies on fetal behavior, new discussions have arisen about the existence of a possible rudimentary psychic apparatus. Questioning the existence of a psychism in the fetus becomes doubly challenging. First, because of the controversy that exists in the field of neuroscience about the studies of epiphenomena. Second, because of the difficulty that psychoanalysis has in accepting the existence of a psychic structure before birth. This study was carried out considering all these controversies and scientific limitations, and for this reason it should be understood as a theoretical hypothesis and an invitation to a broad and transdisciplinary view on the complexity of human behavior. From an extensive review on the development of the nervous system and fetal synaptogenesis, and combining neurophysiological and neurophysical research, it was possible to create a link with the Freudian theory of psychic energy described in the Project for a scientific psychology. From these joints, questions were raised about fetal development, especially in the preterm phase, which would be composed of intense synaptic activities, especially in the somatosensory and thalamocortical regions that would receive exogenous and endogenous stimuli, both acting to generate an accumulation of psychic energy. Thus, it was hypothesized that this intense flow of energy would be the first sign of the development of the primitive psychic apparatus in the fetus. Thus, it was possible to assume that during the preterm period this cathected energy discharge could project directly onto the limbic and motor brain structures and leave unconscious memory traces of intrauterine life experiences. These influences of a psychic nature, together with epigenetic factors, would contribute to the appearance of certain behavioral and neurodevelopmental disorders. Therefore, suggesting an early transdisciplinary approach in at-risk infants exposed to environmental or epigenetic stressors during the gestational period, especially during the synaptic plasticity window, will provide a therapeutic opportunity through psychic reorganization and sensorimotor integration.


Introdução: As pesquisas neurocientíficas têm proporcionado grandes descobertas no que concerne ao entendimento sobre o funcionamento cerebral e seus circuitos neurais. Com os avanços nos estudos sobre o comportamento fetal novas discussões têm surgido acerca da existência de um possível aparelho psíquico rudimentar. Questionar a existência de um psiquismo no feto, torna-se duplamente desafiador. Primeiro pela controvérsia que existe no âmbito da neurociência sobre os estudos dos epifenômenos. Segundo, pela própria dificuldade que a psicanálise tem em aceitar a existência de uma estrutura psíquica antes do nascimento. Este estudo foi realizado considerando todas estas controvérsias e limitações científicas, e por este motivo deve ser entendido como uma hipótese teórica e um convite para uma ampla e transdisciplinar visão sobre a complexidade do comportamento humano. A partir de uma extensa revisão sobre o desenvolvimento do sistema nervoso e da sinaptogênese fetal, e associando as pesquisas neurofisiológicas e da neurofísica, foi possível criar uma articulação com a teoria Freudiana da energia psíquica descrita no Projeto para uma psicologia científica. A partir destas articulações, levantou-se questionamentos sobre o desenvolvimento fetal, especialmente na fase pré-termo, o qual seria composto por atividades sinápticas intensas, especialmente nas regiões somatossensoriais e talamocorticais que receberiam estímulos exógenos e endógenos, ambos atuando para gerar um acúmulo de energia psíquica. Desta forma, criou-se uma hipótese de que este intenso fluxo de energia seria o primeiro sinal do desenvolvimento do aparelho psíquico primitivo no feto. Assim, foi possível supor que durante o período pré-termo esta descarga de energia catexizada poderia se projetar diretamente sobre as estruturas cerebrais límbicas e motoras e deixar traços de memória inconscientes das experiências da vida intrauterina. Seriam estas influências de natureza psíquica em conjunto com os fatores epigenéticos, que contribuiriam para o aparecimento de certos transtornos comportamentais e do neurodesenvolvimento. Sendo assim, sugerir uma abordagem transdisciplinar precoce em bebês de risco expostos a fatores estressores ambientais ou epigenéticos durante o período gestacional, especialmente durante a janela de plasticidade sináptica, proporcionará uma oportunidade terapêutica através da reorganização psíquica e da integração sensoriomotora.

ABSTRACT OBJECTIVE To investigate the association between birth weight and bone mineral content (BMC), and whether this relationship differs between men and women. METHODS A total of 10,159 participants from the ELSA-Brasil cohort were eligible for this analysis. The outcome was the z-score of the ratio BMC (kg)/height (m). The exposure was the low birth weight (< 2.5kg). The magnitude of the associations was estimated by mean differences and their respective 95% confidence intervals (95%CI) using linear regression. All analyses were presented for the total population and stratified by sex. RESULTS Most were women (54.98%), and the mean age was 52.72 years (SD ± 6.6). In the crude model, we observed that low birth weight was associated with a lower mean BMC/height z-score, compared to adequate birth weight (mean difference: −0.30; 95%CI: −0.39 to −0.21), and this effect was stronger in men (mean difference: −0.43; 95%CI: −0.56 to −0.30) than in women (mean difference: −0.31; 95%CI: −0.44 to −0.19). After adjusting for age, sex per total population, race/skin color, maternal education, individual education, and current weight, there was a considerable reduction in the magnitude of the association (total population: −0.10; 95%CI: −0.14 to −0.06; men: −0.13; 95%CI: −0.21 to −0.06; women: −0.13; 95%CI: −0.21 to −0.05). CONCLUSION Low birth weight is related to BMC/height z-score in both sexes with no indication of differences by sex. The magnitude of the associations was attenuated after adjustment for the current weight.

ABSTRACT

Abstract Objective: In this brief review, the authors focus on the effects of gestational exposures to urban air pollution on fetal development and neonatal outcomes. Source of data: In this review the authors used PubMed, Web of Science and SciELO research platforms, analyzing papers from the last 30 years. Summary of the findings: Epidemiological and experimental evidence agree that gestational exposure to air pollution in urban increases the risks for low birth weight, preterm birth, congenital malformation, intrauterine growth restriction, and neonatal mortality. Furthermore, exposures are associated with increased risks for preeclampsia, hypertension, gestational diabetes. Conclusions: Therefore, it is time for greater involvement and engagement of the health sector in the discussion of public policies that may affect the quality of the environment, and that directly or indirectly impact the health of those who were not yet born.

RESUMEN La evaluación del cerebro fetal es un punto imprescindible en el ultrasonido obstétrico, por la gran cantidad de malformaciones que pueden ser diagnosticadas. La guía de ISUOG nos brinda los cortes elementales para la sospecha de la patología cerebral; pero, podemos ampliar y mejorar nuestro ultrasonido con la visualización de estructuras fácilmente reproducibles, tales como el complejo anterior, cuerpo calloso, cisura de Silvio y el cuarto ventrículo. Presentamos algunas herramientas para complementar la evaluación del cerebro fetal.


ABSTRACT The evaluation of the fetal brain is an essential point in obstetric ultrasound due to the large number of malformations that can be diagnosed. The ISUOG guide provides us with the elementary sections for the suspicion of brain pathology; but we can extend and improve our ultrasound with the visualization of easily reproducible structures, such as the anterior complex, corpus callosum, Sylvian fissure and the fourth ventricle. We present some tools to complement the assessment of the fetal brain.

RESUMEN Fundamento: la edad y el crecimiento fetal se determinan mediante la longitud céfalo-caudal o cráneo raquis entre las semanas cinco y 10 de la gestación y después se recurre a una combinación de medidas de otras variables biométricas. Objetivo: identificar correlación de las variables biométricas y condición trófica al nacer con la longitud cráneo raquis del primer trimestre. Métodos: se realizó una investigación con diseño analítico longitudinal retrospectivo en la provincia Villa Clara, se recurrió al período comprendido entre enero de 2013 a octubre de 2018. La población estuvo conformada por 6 050 gestantes. La muestra se realizó a través de un muestreo no probabilístico intencional por criterios, constituida por 3 910 gestantes. Se obtuvieron datos de libros, registros de las consultas de genética de áreas de salud seleccionadas. En el análisis, se utilizaron el coeficiente de correlación lineal de Pearson, de Rho de Spearman y diagramas de dispersión. Resultados: existió correlación entre longitud cráneo raquis con las variables y con la condición trófica del recién nacido. Los gráficos que definieron la trayectoria de las variables biométricas desde el segundo trimestre hasta el parto mostraron compactación de los valores hasta la semana 25 y dispersión entre la 25 y la 30 con rasgos diferenciales en las mismas. Conclusiones: la correlación encontrada de la longitud cráneo raquis con las variables biométricas y con la condición trófica del recién nacido demuestra la utilidad del empleo de esta variable en la evolución del embarazo tanto por el obstetra como por el médico general integral; en lo particular en gestantes en las que se sospeche alguna alteración del crecimiento. Los resultados obtenidos motivan a la profundización al subdividir la muestra en subgrupos como las afectadas de diabetes mellitus o hipertensión arterial u otra, porque no existe en la literatura información sobre la misma.


ABSTRACT Background: age and fetal growth are determined by head length or cranial-spinal length between weeks 5 and 10 of gestation and then a combination of measurements of other biometric variables is used. Objective: to identify the correlation of biometric variables and trophic condition at birth with the cranial-spinal length of the first trimester. Methods: an investigation with a retrospective longitudinal analytical design was carried out in Villa Clara province; it was appealed in the period from January 2013 to October 2018. The population consisted of 6050 pregnant women. The sample was made through an intentional non-probabilistic sampling by criteria, consisting of 3 910 pregnant women. Data were obtained from genetics consultation record books from selected health areas. In the analysis, Pearson's linear correlation coefficient, Spearman's Rho, and scatter diagrams were used. Results: there was a correlation between the cranial-spinal length with the variables and with the trophic condition of the newborn. The graphs that defined the trajectory of the biometric variables from the second trimester to delivery showed compaction of the values ​​up to week 25 and dispersion between week 25 and 30 with differential features in them. Conclusions: the correlation found of cranial-spinal length with biometric variables and with the trophic condition of the newborn demonstrates the usefulness of the use of this variable in the evolution of pregnancy by both the obstetrician and the comprehensive general practitioner; particularly in pregnant women in whom some growth alteration is suspected. The results obtained motivate further study by subdividing the sample into subgroups such as those affected by diabetes mellitus or arterial hypertension or another, particularly because there is no information on it in the literature.

ABSTRACT Early weaning can predispose the offspring to greater risk of developing chronic diseases in adulthood. It is believed that the consumption of functional foods is able to prevent these effects. The aim of this study is to evaluate the effects of maternal and postnatal cocoa powder supplementation on body mass, metabolism, and morphology of the prostate of early weaned Wistar rats. The animals were divided into four experimental groups according to lactation time (21 or 18 days, n=6, each) as follows: control group (C), cocoa control group (CCa), early weaning group (EW), and cocoa early weaning group (EWCa). The animals were euthanized at 90 days of age. Serum biochemical analysis and prostate histomorphometric evaluation were performed. The animals supplemented with cocoa powder were heavier than their respective controls (p <0.05), although with no difference in food intake among the groups. Likewise, these same groups showed a reduction in the serum glucose in relation to C and EW groups (p <0.0001). With respect to the prostate, there was no difference in smooth muscle and lumen area densities, while the EW group had a lower epithelial height and a higher percentage of mast cells than the C group (p <0.05). On the other hand, the EWCa group managed to reverse these parameters, leveling with the controls. Early weaning resulted in hyperglycemia and important morphological changes in the prostate. In contrast, dietary supplementation with cocoa powder attenuated these effects on the metabolism and prostatic histoarchitecture, proving to be a good nutritional treatment strategy.

Abstract Objective: To investigate the association between birth weight and excess weight among students aged 6-14 years, adjusted for life course confounding factors. Methods: Cross-sectional study with 6-14-year-old schoolchildren in 2010; 795 school children from two public schools. In addition, a sub-sample was selected using a case-cohort study approach. Sociodemographic, breastfeeding, food introduction, previous weight gain, family history, current clinical and behavioral variables as well as maternal variables related to pregnancy, were collected. Multivariable weighted logistic regression was used to evaluate the association between birth weight and overweight. All prevalent cases of overweight (n = 160) were selected to compose the case group and a random sub-sample of all students participating in the study (n = 276 students, of whom 88 were cases) were the control group. Results: An unadjusted 6% increase in the excess weight prevalence ratio (p-value = 0.004) was found for each 100 g increase in birth weight. With adjustment for age, sex and behavioral variables (models 1 and 2), the association of birth weight with excess weight was positive and statistically significant, but it was no longer significant in the final model (model 3) when clinical variables were considered. Conclusions: Although some of the secondary associations were statistically significant, we could not identify a significant association between birthweight and excess weight in adolescents.

RESUMEN Introducción: Aún persisten controversias en los eventos de la morfogénesis cardiovascular y una ausencia, casi total, de parámetros morfométricos en las fases iniciales de su desarrollo. Objetivos: Determinar la razón miocardio no compactado/miocardio compactado (NC/C) en ambos ventrículos y la evolución cronológica de esta razón en el período estudiado. Método: Se realizó un estudio descriptivo, transversal, con 18 embriones humanos pertenecientes a la Embrioteca de la Universidad de Ciencias Médicas de Villa Clara (Cuba) clasificados entre los estadios 17 y 23 de Carnegie. Se determinó el índice NC/C, el cual no es más que el cálculo matemático de la razón entre las porciones no compactada y compactada por espécimen y por estadios. Resultados: Los resultados de la aplicación de este índice en el ventrículo derecho de los embriones son: 7,17; 4,26; 3,12; 2,79; 2,36; 2,84 y 2,10 en los estadios de Carnegie 17, 18, 19, 20, 21, 22 y 23, respectivamente. En estos mismos especímenes se obtuvo como resultado en el ventrículo izquierdo: 5,0; 3,80; 2,68; 2,18; 2,50; 2,01 y 1,56, igualmente organizado por estadios. Conclusiones: Los índices NC/C obtenidos sustentan cuantitativamente que la compactación del miocardio ventricular avanza en los estadios evaluados; sus valores, mayores en el vértice, denotan que es posible que aún no haya concluido en esta zona.


ABSTRACT Introduction: Controversies still persist regarding the events of cardiovascular morphogenesis and an almost total absence of morphometric parameters in the initial phases of its development. Objectives: To determine the non-compacted to compacted (NC/C) myocardium ratio in both ventricles and the chronological progression of this ratio in the period studied. Method: A descriptive, cross-sectional study was carried out with 18 human embryos belonging to the Embryoteca of the Universidad de Ciencias Médicas de Villa Clara (Cuba) classified between Carnegie stages 17 and 23. The NC/C ratio -which is simply the mathematical calculation of the ratio between the non-compacted and compacted portions per specimen and per stage- was determined. Results: The application of this ratio in the right ventricle of the embryos obtained the following results: 7.17; 4.26; 3.12; 2.79; 2.36; 2.84 and 2.10 in Carnegie's stages 17, 18, 19, 20, 21, 22 and 23, respectively. In these same specimens, the left ventricle yielded the following results: 5.0; 3.80; 2.68; 2.18; 2.50; 2.01 and 1.56, also organized by stages. Conclusions: NC/C ratios obtained quantitatively support a progression of the ventricular myocardial compaction in the evaluated stages; their higher values at the apex denote that it may still be incomplete in this zone.

ABSTRACT Objective The spinal cord extends from the foramen magnum to the sacrum in the human fetus at the beginning of the 2nd quarter. However, the medullary cone is located at or above the level of the second lumbar vertebra at birth. The objective is to determine the difference between the rates of longitudinal growth of the spinal cord and the spine in human fetuses from the 13th to the 22nd week of gestation (WoG) using magnetic resonance imaging (MRI). Methods Descriptive observational cross-sectional study of 24 stillbirths (13 ♂, 11 ♀), between the 13th and 22nd WoG, using spinal MRI. We recorded spine and spinal cord lengths in millimeters from the foramen magnum to the coccyx for the former and to the medullary cone for the latter. We identified the position of the medullary cone according to vertebral level and its correlation with the gestational age and the literature. Results The spinal cord increased in length from 50 to 93 mm, the spine from 57 to 137 mm, and the medullary cone rose from S1 to L2. The rate of growth was 1.2 mm/day for the spine and 0.6 mm/day for the spinal cord. Conclusions Discordance in the rate of normal longitudinal growth of the spine and spinal cord caused the medullary cone to rise from S1 level to L2 in the second trimester of pregnancy. These results allow an understanding of normal development and certain congenital malformations. Level of evidence IV; Case series.


RESUMO Objetivo A medula espinhal (ME) estende-se desde o forame magno até o sacro no feto humano no início do 2º trimestre. No entanto, ao nascimento, o cone medular localiza-se no nível da segunda vértebra lombar ou acima. O objetivo é determinar as diferenças na taxa de crescimento longitudinal da ME e da coluna vertebral (CV) em fetos humanos da 13a à 22a semana de gestação (SG) por meio de ressonância magnética (RM). Métodos Estudo observacional transversal descritivo em 24 natimortos (13 ♂, 11 ♀), com idades entre 13ª e 22ª SG, por RM da CV. O comprimento da CV e da ME foi registrado em milímetros, desde o forame magno até o cóccix na CV e até o cone na ME. Identificou-se a posição do cone de acordo com o nível vertebral, sua correlação com a idade gestacional e com literatura. Resultados O comprimento da ME aumentou de 50 para 93 mm, a CV de 57 para 137 mm e o cone medular subiu de S1 para L2. O ritmo de crescimento foi de 1,2 mm/dia para a CV e de 0,6 mm/dia para a ME. Conclusões A discordância no ritmo do crescimento longitudinal normal da CV e da ME fez com que o cone medular subisse do nível de S1 até L2 no segundo trimestre de gravidez. Os resultados permitem compreender o desenvolvimento normal e certas malformações congênitas. Nível de evidência IV; Série de casos.

Sin embargo, el cono medular se ubica a nivel de la segunda vertebral lumbar o por encima en el momento del nacimiento. El objetivo es determinar las diferencias en el ritmo de crecimiento longitudinal de la ME y columna vertebral (CV) en fetos humanos desde la 13ª hasta la 22ª semana de gestación (SG) mediante resonancia magnética (RM). Métodos Estudio descriptivo observacional transversal en 24 mortinatos (13 ♂, 11 ♀), con edades entre la 13ª y 22ª SG, mediante RM de CV. Se registró la longitud de CV y ME, en milímetros, desde el foramen mágnum al coxis en la primera y hasta el cono en la segunda. Se identificó la posición del cono según el nivel vertebral y su correlación con edad gestacional y literatura. Resultados La ME incrementó su longitud de 50 a 93 mm, la CV de 57 a 137 mm y el cono medular ascendió desde S1 hasta L2. El ritmo de crecimiento fue de 1.2 mm/día para la CV y de 0.6 mm/día para la ME. Conclusiones La discordancia en el ritmo de crecimiento longitudinal normal de la CV y ME determinó que el cono medular ascienda desde el nivel S1 hasta L2 en el segundo trimestre de gestación. ABSTRACT OBJECTIVE: To analyze the effects of early determinants on adolescent fat-free mass. METHODS: A cohort study with 579 adolescents evaluated at birth and adolescence in a birth cohort in São Luís, Maranhão. In the proposed model, estimated by structural equation modeling, socioeconomic status (SES) at birth, maternal age, pregestational body mass index (BMI), gestational smoking, gestational weight gain, type of delivery, gestational age, sex of the newborn, length and weight at birth, adolescent socioeconomic status, "neither study/nor work" generation, adolescent physical activity level and alcohol consumption were tested as early determinants of adolescent fat-free mass (FFM). RESULTS: A higher pregestational BMI resulted in higher FFM in adolescence (Standardized Coefficient, SC = 0.152; p < 0.001). Being female implied a lower FFM in adolescence (SC = −0.633; p < 0.001). The negative effect of gender on FFM was direct (SC = −0.523; p < 0.001), but there was an indirect negative effect via physical activity level (SC = −0.085; p < 0.001). Women were less active (p < 0.001). An increase of 0.5 kg (1 Standard Deviation, SD) in birth weight led to a gain of 0.25 kg/m2 (0.106 SD) in adolescent FFM index (p = 0.034). Not studying or working had a negative effect on the adolescent's FFM (SC = −0.106; p = 0.015). Elevation of 1 SD in the adolescent's physical activity level represented an increase of 0.5 kg/m2 (0.207 SD) in FFM index (p < 0.001). CONCLUSIONS: The early determinants with the greatest effects on adolescent FFM are gender, adolescent physical activity level, pregestational BMI, birth weight and belonging to the "neither-nor" generation.
